How to select & export each feature of a large fc into its own fc

Is there a tool or something simple I can attempt in model builder to let me "explode" the features (like data driven pages does to pdf) - to export each individual feature into its own feature class/shapefile?

I need to provide the data like this to import into the CodeRed notification map. If I was doing this manually, I would go to the layer's table and see the name for each neighborhood, select a row - hit Export Data on the TOC, rename the new shapefile name to the neighborhood - and hit export.
