Arcgis 10.1 Intersect tool return unexpected result

Task: intersect point feature with polygon feature by both runing the tool and in modelbuilder
Processing: get output layer without any error messages
Issue: some records return wrong data

Check: no obvious geometry/topology problem

Has anyone ever encountered this issue, any tips to avoid this problem or check the results efficiently?
